I care that he looks healthy and with a voice that can still pack a punch. Hair/no hair, tattoo/no tattoo I don't care. He is the Metal God and should do anything he wishes to do  (especially in the face of unwarranted criticism and overall bullshiticism.)
My favorite look of him has always been the one he had during the Painkiller tour, but he has managed to keep himself fresh in terms of song writing and visual since then. IMO he should shed a lot of the latest outfits, not because they're not cool but because it would give him so much more freedom to move around.
